Terms of Use

These Terms of Use apply to the weight loss quizzes on ivee.clinicquiz.com and ivee-weight-loss.vercel.app (the “Quiz Site”), operated by iVee Restorative Care (“iVee,” “we,” “us”). By using the Quiz Site, you agree to these terms.
1. Who can use the Quiz Site
You must be 18 or older. The services described are provided in person at our office at 290 W Center St, Spanish Fork, Utah.
2. Not medical advice
The quizzes help us understand your goals and schedule a visit. They are not a medical evaluation, diagnosis, or prescription, and your answers do not create a patient–provider relationship. Do not use the Quiz Site for emergencies; call 911.
Any treatment, including any prescription medication, is provided only if a licensed provider decides after an in-person evaluation that it is appropriate for you. Not everyone qualifies, and no specific treatment is promised.
3. The $49 Weight Loss Assessment
The assessment costs $49 and includes a personalized weight loss assessment, a free 3D body scan, and a free B12 boost. The B12 boost is given only when clinically suitable; it is not a weight loss treatment and does not promise more energy. Ongoing programs, medications, and other treatments are separate and are priced and discussed before you agree to them.
4. The 90-Day Weight Loss Challenge
The challenge price, eligibility, judging criteria, tie-break process, and prize details are set out in the official challenge rules, which iVee will give you before you enroll. The money-back prize applies only to the participant selected as the top health changer under those rules. It is not a refund or guarantee for other participants.
5. Booking and contact
Submitting the quiz is a request to be contacted. It does not book an appointment or enroll you until you receive a confirmation from iVee. No payment is taken on the Quiz Site.
6. Text messages
By submitting your phone number, you agree that iVee may call or text you about your request and visit. Message frequency varies. Message and data rates may apply. Reply HELP for help or STOP to opt out. See our Privacy Policy.
7. Results, reviews, and photos
Individual results vary and are not guaranteed. Reviews are quoted from iVee’s Google Business Profile. Before-and-after photos show individual patients’ own experiences; your results may differ.
8. Acceptable use
Provide accurate information, use only your own contact details, and do not misuse or interfere with the Quiz Site.
9. Intellectual property
The Quiz Site’s content, logo, and design belong to iVee or its licensors and may not be copied without permission.
10. Disclaimers and limitation of liability
The Quiz Site is provided “as is.” To the extent allowed by law, iVee is not liable for indirect or consequential damages arising from use of the Quiz Site. Nothing in these terms limits liability that cannot be limited by law.
11. Governing law
These terms are governed by the laws of the State of Utah.
12. Changes and contact
