Privacy Policy

This Privacy Policy explains how iVee Restorative Care (“iVee,” “we,” “us”) handles information collected through the weight loss quizzes on ivee.clinicquiz.com and ivee-weight-loss.vercel.app (the “Quiz Site”). It covers only the Quiz Site.
1. Information we collect
- Contact details you enter: first name, last name, mobile phone number, and email address.
- Quiz answers: your age range, weight goals, what you have tried, your interest in medical weight loss support, timing, and whether you can visit our Spanish Fork office. Some answers relate to your health.
- Visit source: advertising parameters in the page link (such as utm_source, utm_campaign, and fbclid) that tell us which ad or link brought you here.
- Technical data: our hosting provider automatically records standard request data such as IP address, browser type, and pages requested, for security and reliability.
Quiz answers you choose not to finish are not sent to us. Your answers are only sent when you submit your contact details on the last step.
2. How we use your information
- To contact you about the offer you asked about and to schedule your visit.
- To prepare for your visit, so the team understands your goals before you arrive.
- To send appointment confirmations, reminders, and other messages related to your request.
- To measure which ads and pages work, using the visit source parameters and the Meta Pixel.
- To protect the Quiz Site and comply with the law.
3. Text messages (SMS)
By submitting your phone number, you agree that iVee Restorative Care may call or text you about your request and visit. Message frequency varies. Message and data rates may apply. Reply HELP for help or STOP to opt out at any time.
We do not share or sell your mobile number or text-messaging consent with third parties or affiliates for their marketing or promotional purposes. We may share it with service providers that help us send messages on our behalf.
4. How we share information
- Service providers that operate the Quiz Site and our follow-up for us, including website hosting (Vercel), our customer relationship and messaging platform (GoHighLevel / LeadConnector), our online scheduling calendar, and Meta (Facebook) for ad measurement. They may use your information only to provide services to us.
- Our clinical team at iVee Restorative Care, to prepare for and provide your care.
- As required by law, or to protect the rights, property, or safety of iVee, our patients, or others.
We do not sell your personal information.
5. Health information
Quiz answers are collected for scheduling and marketing follow-up, before you become a patient. Once you receive care at iVee, your medical records are protected under HIPAA and our Notice of Privacy Practices, which is available on request at info@iveerestorativecare.com.
6. Cookies and browser storage
The Quiz Site uses the Meta Pixel, which may set cookies and tells Meta (Facebook) when a page is viewed and when a quiz is submitted. Your quiz answers and contact details are not sent to Meta through the pixel. The Quiz Site also stores the visit source parameters in your browser’s session storage until you close the tab. The scheduling calendar shown after the quiz is provided by a third party and may set its own cookies. See our Cookie Policy.
7. Security and retention
Information is sent over encrypted connections (HTTPS) and is accessible only to authorized staff and service providers. We keep it for as long as needed for the purposes above or as required by law. No method of transmission or storage is completely secure.
8. Your choices
- Reply STOP to any text to stop texts. Use the unsubscribe link in any marketing email.
- Email info@iveerestorativecare.com to ask for a copy of your information, or to correct or delete it.
9. Children
The Quiz Site is for adults 18 and older. If you select “Under 18,” the quiz ends and no contact details are requested. We do not knowingly collect information from children.
10. Changes
We may update this policy. The effective date above shows the latest version.
